PLEASE REMEMBER TO ORDER YOUR MEDICATION IN PLENTY OF TIME, ESPECIALLY IF YOU ARE GOING AWAY.
There are a number of ways in which you can request a repeat prescription.
Via your local pharmacy - in person or via telephone. The pharmacy will then order your medication from the surgery on your behalf.
In Person - please tick the items you require on your repeat slip and put into the box just inside the main surgery entrance.
Post - please post your prescription request to the surgery enclosing a stamped addressed envelope if you would like us to post your prescription back to you.
Fax - our fax number is 01803 852637
Online - you can request your medication online if you are registered for Patient Access. Please ask at reception for more details or click on the link.
Email - please email a full list of the medication you require, including your name and date of birth to L83078.email@example.com
If you do not have a repeat prescription slip or have a specific medication query then the Surgery will be happy to help you in person or over the telephone if required.
Please ensure the doctor or nurse has approved the prescription before requesting it.
PLEASE REMEMBER YOU MUST PROVIDE A MINIMUM of 2 working days NOTICE FOR ALL ROUTINE PRESCRIPTION REQUESTS.
Prescription requests received on Fridays or over the weekend will be ready for collection the following Tuesday after 4:00pm.
Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificiates (PPCs) offer real savings for people who need extensive medication.
NHS Low Income Scheme (claim form HC1)
The NHS Low Income Scheme Provides help for those who don't automatically qualify for help with health costs such as prescription charges, but who find it difficult to pay. Anyone can apply using claim form HC1 as long as they don't have savings or capital of more than £16,000 (£23,250 for those permanently in a care home).

Prescription Pre-Payment Certificate (application form FP95)
If you pay for your prescriptions you can save money with a pre-payment certificate (PPC). A 12 month PPC can be paid for either up front in full or by 10 monthly direct debit instalments of £10.40.

Prescriptions will be ready after 4pm, 2 working days after requested.
This means that a prescription requested on Friday will be ready after 4pm the following Tuesday.
